CME/CE Program Developers:

If you are a Program Developer (PD) you MUST do the following:

  1. Submit a PRF (Program Request Form) via email at least six weeks before a course begins or before a brochure needs to be printed. You will be notified if any corrections/revisions are needed. The deadline is one week for Program Developers to return revised/corrected PRFs.
     
    1. a. Obtain an instructor’s/presenter’s CV and send it to the Extension Division/CME Coordinator via e-mail
      b. When a bibliography is required please send via e-mail (see bibliography format)
      c. Developer Disclosure form and Confirmation Form need to be signed and sent in no later than the program’s date. 
      d. Faculty Disclosure Form needs to be signed by the presenter and/or moderator and/or discussant before the program begins.
       
  2. At the completion of a course/program, the Program Developer/Instructor will distribute evaluation forms to participants (these will be given to PDs with sign-up sheets at the beginning of the course/program). For programs/courses that take place over several weeks/months, evaluations will be completed on-line using survey monkey. A SFCP Documentation of Attendance form will be sent out to those participants who have completed the survey monkey and paid their CE/CME fees. PDs should also inform participants that they may receive a post program evaluation via survey monkey three months after the end of the course so that we can assess enduring learning/changes. (Please note, for psychologists to obtain full credit for a longer course, they must attend 80% of the classes).
     
    1. Once the CME/CE Coordinator receives all evaluation forms, he/she will send the PD the data from the evaluations. PDs will then be able to complete a Summary Evaluation of the course. PDs should be sure to include this data in the Summary Evaluation. The Summary Evaluation MUST be completed within 1 month of receiving the evaluation data (example of a good summary evaluation).
       
  3. Please be sure to communicate the information in the Letter to Presenter to all invited presenters.  You may either send them this letter or communicate the information to them verbally.


Filling out a PRF:

EDUCATIONAL OBJECTIVES: MUST focus on observable and measurable learning outcomes. The focus in educational objectives is on the learner and must contain ACTION verbs such as:

List, describe, discuss, explain, apply, demonstrate, analyze, utilize, assess, compare, critique.

DO NOT use objectives like: know, understand, learn, appreciate, become familiar with.

It is crucial that this part of the PRF be filled out according to these instructions (please also see example).

NEEDS ASSESSMENT: This aspect of the PRF should focus on why this course is being offered by SFCP. Courses should, in some way, facilitate a change in participants’ competence and performance as clinicians. Programs should help attendees acquire new ideas and approaches to patient care that they can bring to their practices. Hence, in this section, please address demonstrated needs, expressed needs and presumed needs from this perspective. Please indicate why this program will be useful to attendees (click on example of a good Need Assessment).

DESIRABLE PHYSICIAN ATTRIBUTES: Please indicate which desirable physician attributes were considered in the development of your course/program. Refer to ACGME Core Competencies.

CULTURAL AND LINGUISTIC COMPETENCY: Please answer both questions. Be sure to write down the relevant course objective that addresses the element of cultural competency in your program. Please refer to the informational section on Cultural and Linguistic Competency on the SFCP website or call the PRF Reviewer for clarification on these questions.
